How to Add Units or Hours to a Care Plan in GuidingCare

Overview

This article explains how to record service units or hours on a member’s Care Plan in GuidingCare. Hours and units are entered using the Actual Duration field when performing an activity, and are linked to the appropriate Service Code (T1017 HT) for Tailored Care Management billing and reporting.


Steps to Add Units or Hours During an Activity

  1. From your calendar, locate the scheduled activity linked to the Care Plan.

  2. Hover over the activity and click Perform

  3. Complete the required activity details.

  4. Under Activity Outcome, enter the following:

    • Actual Duration → Enter the time spent with the member (e.g., 0.5 for 30 minutes, 1 for one hour).

    • Place of Service → Select from the dropdown (e.g., Community, Home, Office).

    • Service Code → Select T1017 HT (required for all TCM activities).

  5. When done, click Save & End Script.

  6. The Capture Activity Outcome screen will appear. Review and confirm your entries:

    • Verify Actual Duration reflects correct time or units.

    • Confirm Service Code = T1017 HT.

  7. Click Save and Proceed to complete the entry.


Adding Units or Hours When Editing or Adding a Goal (OGI)

  1. Navigate to Member Details → Care Plan.

  2. Select the OGI (Opportunity, Goal, Intervention) you wish to update.

  3. Click Edit.

  4. Enter or update:

    • Status (Recommended, Current, or Completed)

    • Assign Owner (Responsible staff)

    • Start Date and Target Date

    • Notes (if applicable)

  5. Click Update. When prompted to add an activity, select Yes.

  6. In the Add Activity window:

    • Select Activity Type and Contact Type.

    • Enter Actual Duration (hours/units).

    • Add any relevant comments.

    • Click Add and Close.

Tips

  • Always verify the Service Code before saving.

  • The Actual Duration field supports decimals for partial hours.

  • If edits are needed after saving, you can re-open the Activity Outcome screen to make corrections.

  • Only signed (Current) OGIs allow activities and units to be added.


Troubleshooting

  • Field greyed out? Ensure the OGI is marked as Current (signed).

  • Unable to save? Verify all mandatory fields (Condition, Goal Group, Goal, and Intervention) are filled.

  • Duration missing in billing report? Re-check that the activity is marked Completed with a valid service code.
