Steps to Send Care Plans for Signatures
Access the Care Plan
Navigate to the Care Plan section in GC.
Click on Full Care Plan to open a new dialog box.
Select Portions for Printing
Choose the specific sections of the care plan that need to be included in the printed document.
Add Signatories
Scroll down to the Signature Log section at the bottom of the page.
Click the “+” button to add each care team member who needs to sign the document.
If a member or care team member is unable to sign electronically, select their name and leave the e-signature field blank.
Generate a PDF
Once all necessary signatories have been added, generate the care plan as a PDF document.
Capture Signatures
If e-signatures were not captured in GC, use an external method to obtain signatures:
DocuSign: Upload the PDF to DocuSign and request electronic signatures.
Paper Signatures: Print the document and obtain physical signatures.
Additional Notes
Ensure all required care team members are included before finalizing the document.
Once signed, upload the completed document back into GC for record-keeping.
If any changes are made to the care plan, repeat the steps to ensure all necessary approvals are captured.
