Care Plan Note Not Displaying in Care Plan History
Purpose
To provide guidance on where Care Plan notes are stored in GuidingCare and how to properly locate them when they are not visible in the Care Plan History.
Audience
Care Managers, Supervisors, and Support Staff using GuidingCare.
Problem Statement
Users report that Care Plan notes do not appear in the Care Plan History section under Action >> History.
Resolution / Expected Behavior
This is expected system behavior.

Care Plan notes are not stored in Care Plan History.

Instead, Care Plan notes are accessible under Activity Record >> Notes.

Notes can be filtered by Note Type = “Care Plan.”

Notes can also be located via the Context Box → Note option.
Steps to Resolve / View Care Plan Notes

Login to GuidingCare.

Access the member record.
Navigate to:

Member Info
-
Activity Record >> Notes
-
Use filters to locate Care Plan notes:

Sort by Note Type: Care Plan.

Or, click the Context Box → select Note.

Result
Care Plan notes will display in Activity Record >> Notes but will not appear in Care Plan History. This is working as designed.
