Completing Member Assessments - Tailored Plan

Completing assessments accurately and efficiently is essential for effective member care management. This article provides a clear, step-by-step guide on scheduling, performing, and finalizing assessments within the CHN Care Management system.

Step-by-Step Guide

Step 1: Scheduling the Assessment

  1. Open your CHN Care Management calendar.
  2. Select a date by double clicking that date and time slot suitable for both you and the member.

3.      Create an assessment activity by entering the required member details.  

o   Select the Activity Type as "Assessment - Initial."

  • Ensure you select the appropriate assessment script based on the member’s age group (e.g., pediatric, adolescent, adult, geriatric).

 

 Step 2: Performing the Assessment

  1. On your calendar, hover over the scheduled assessment activity.
  2. A pop-up will display with member details and activity options.
  3. Click Perform to initiate the assessment.

 The assessment script opens with some pre-populated information. Complete any remaining details:

o   Select Activity Outcome from the dropdown

o   Enter the Actual Duration.

o   Choose the Place of Service.

o   Ensure the Service Code (typically T1017 HT) is selected correctly.

Step 3: Scroll Down to select the Assessment Interface

  1. Select your preferred mode:

o   Single Screen: Recommended for first assessments. Choose "From member demographics only."

o   Workflow Mode: Guides step-by-step through the assessment.

o   Navigate through questions using:

o   Auto-opened questions as you progress.

o   Clicking directly on questions.

o   Using the icon to move sequentially.

  Step 4: Completing the Assessment Questions

o   Complete all mandatory questions from initial demographic information to guardian details.

o   If a new Care Team member is mentioned, add via Quick Links. (see image below)

o   Identify other services or assistance needs the member might require.

 

Step 5: Finalizing the Assessment

  1. Once all questions are answered, click Save & End Script.

If any questions are unanswered, the system alerts you. Return and complete the missed question(s).

  

  1. Otherwise confirm completion by clicking OK.

  1. Verify the pre-populated information in the Capture Activity Outcome form, adjusting if necessary.
  2. Unclick Review Care Plan checkmark if it's the first assessment (the Care Plan will now auto-generate from the responses).
  3. Click Save and Continue.

 

Step 6: Reviewing the Completed Assessment

o   View the completed assessment in full by scrolling through the finalized script.

o   Confirm all chosen options and enter details.

Common Issues & Solutions

o   Missed Questions: The system will alert you. Click OK to revisit and complete missed questions.

o   Incorrect Pre-Populated Data: Always verify pre-populated fields for accuracy and update as needed.

o   Activity Outcome Errors: Ensure the correct outcome and duration are selected to reflect accurate reporting.

Tips for Efficiency

o   Regularly save progress during lengthy assessments using Save and Continue.

o   Use Save For Later if interruptions occur, ensuring no loss of information.

o   Familiarize yourself with question flow to speed up the assessment process.
