How to View Active Cases/Clients
You will receive members/clients in two ways:
1. Members will be assigned to you by a supervisor. Members who are assigned to you will show up in a list under My Members on the upper left side.
2. Members will be referred to you. Any member referrals will show up under Request Received on the lower right side. You must accept the referral in order for the member to be in your caseload and show up under My Members.
To access your caseload:
- Find the My Members tab on the Care Coordination Home Page

- Your clients will be displayed in a list here. You can filter by Assigned Type, Enrollment Status, and Risks.

- Click the member’s last name to access the client’s record.
- You can always search for your client or any other contact view the search box on the upper right hand corner of the screen.

You may also have referrals. Any members being referred to you will be displayed under Request Received. You must accept each referral in order for the member to show up under My Members.
To accept referrals:
Go to Request Received
Make sure the search values are CM and Member
Members in this list have been referred to a specific care manager.
Once you identify the member you want to accept, check the box to the left of the member’s last name.
Choose Accept in the drop down list
