Scheduling the Initial Outreach
The Care Manager has reviewed the case and is ready to schedule the initial contact with the prospective member to introduce the service, understand the member’s needs, and schedule the initial assessment.
Under Required Activities, on the right hand side, you will see Thumbs and Plus signs.
- Plus signs allow you to schedule the activity:
- Clicking on a thumb will take you to perform the pending script for this activity:
- Clicking on the Plus Sign, will open an Add Activity screen where you can schedule the activity:
2. A pop up window will open to Add Activity
3. Type will be defaulted to Scheduled and greyed out.
4. Script will be defaulted to Contact Script.
5. Activity Type will be defaulted to Initial Outreach.
6. You can edit Contact Type:

7. Select Priority.
8. Select the Scheduled Date/Time for this activity: 9. Enter any comments for this scheduled activity.
10. Do not use Member Portal Activity.
11. Click on Add or Add and Close:
12. If you are scheduling an activity past its due date, you will get this pop up:
13. Click Yes to proceed to schedule 14. If you look at your calendar, you will now see the activity scheduled for the time & day you chose:
15. When the appointment time arrives, you can record the activity directly from your calendar clicking on either the Initial Outreach or the thumb in the calendar item.
